Obviously, there are a myriad of factors taken into consideration by Google when they determine the page rank of your website, so it can be hard to determine what the problem is. The best idea is to concentrate on what Google does love: back links and great content. If your page rank is low, or recently dropped, add a blog to your website. Be sure it shares the domain name with your main page as that will boost the rank of your entire website. Also consider sites which you can trade links with, plus include back links within your own website, such as a "Similar Content" or "Suggested Reading" section at the bottom of each page. Don't include too many links, though, as Google frowns upon that.  Should my key phrases be underlined, in bold or italics? The fact is that it is unlikely that Google pays attention to these HTML or CSS tags at all. It is too easy to put those tags specifically on your keywords, so Google wouldn't put any weight in them when ranking your page. Use underline sparingly as it will confuse readers who expect the text to be a link. Bold should only be used to put emphasis on words, and italics only when it seems natural to use it. Bolding within articles or blog posts should be used to make the text itself easy to scan, the same idea as using bullet lists or headings. Try to keep it natural and Google is unlikely to punish you for your SEO techniques.  Why am I not getting a lot of traffic from the key phrases I chose? The fact is that selecting the wrong keywords will doom you. Recheck your words using Google Keywords Tool and you may find out that how much search activity they receive has gone down, or that other sites are now targeting them. Continually try out new key phrases and monitor those you already have in use to keep your site at the top of the results page.  Yesterday, my site was ranked highly. Today, it's not even on the front page! What gives? Google changes its algorithms quite often, and those using search engine optimization techniques which are grey or black hat will be penalized when it does. Publish unique, interesting content and your site will remain at the top of the results regardless of algorithm changes.  Continuing to learn about search engine optimization is the only way to continually build your rank on search engine results pages.
